Each plan is valid for one year and is non-refundable
Our discount plans are exclusive and cannot be used in conjunction with any other discounts.
Discounted fees for dental services become applicable only once the enrollment fee is fully paid.
Full payment is required at the time of service. Otherwise, the treatment will be billed at regular fees.
The discount will be reduced if you are using Care Credit.
The plans are solely for use at Aspire Dental Wellness and do not cover services outside our clinic.
You may cancel your membership at any time by submitting a written request; however, please note that the plan is non-refundable.
Patients diagnosed with periodontal disease are not eligible for routine cleanings under the Aspire Dental Wellness Basic Membership Plan, as active treatment is required once the condition is identified.
